The 2008 Kindergarten registration will be on March 1, 2, 3, 4, and 5, 2010.   Kindergarten requirements are as follows:
 

	Original Birth Certificate - Child must be 5 years of age before October 1st.
	Complete immunization against D.P.T. - a series of four is required with one of the doses given on or after the fourth birthday.
	Complete immunization against Polio - a series of three Tri-valent with one of these doses given on or after the fourth birthday, or any four doses.
	Immunization against Measles (2 doses given after one year of age or written laboratory evidence of immunity).
	Immunization against Rubella (after one year of age or written evidence of immunity).
	Immunization against Mumps or written evidence of immunity.
	Immunization against Hepatitis B (3-series).
	Immunization against Varicella (vaccine given on or after the first birthday of proof of disease immunity with medical documentation).
	Record of Physical Exam no more than 365 days prior to entry into school.
	Doctor's or Health Department Certificate is required as written proof that the above were completed
	Child's social security number

NOTE:  State law prohibits the acceptance of any child who does not meet these requirements prior to the first day of entering school in September.  This requirement is strictly enforced in our district.
