Required Immunizations

Enrollment Requirements
Upon initial enrollment in Jefferson Township School District, all new students must present an original birth certificate bearing a raised seal (photocopies are not accepted) along with proof of immunizations.

Where applicable, students may be exempt from immunization requirements for medical or religious reasons, provided that appropriate waivers and supporting documentation are submitted.

Students entering from outside the United States must also present State Board of Health clearance documentation before enrollment can be completed.

Important Notes for Families

• Proof of immunization records must be an official document (i.e. document from primary practitioner or medical facility, NJIIS record, or record from prior school).
• A history of chickenpox disease may count in place of the Varicella vaccine.
• Students who present documented laboratory evidence of measles, mumps, rubella, or hepatitis B immunity shall not be required to receive that vaccine. 
• New Jersey regulations allow valid medical and religious exemptions.
• During outbreaks of vaccine-preventable diseases, students without proof of immunity may be excluded from school.

Helpful Reminders

• Schedule annual physicals and vaccine visits early.
• Send updated immunization records to the school nurse.
• Contact your child’s healthcare provider with vaccine questions.
